To address the high burden of noncommunicable diseases (NCDs) in India, Swasthya Sankalp project has been launched across Gujarat, Maharashtra, and Karnataka to strengthen CPHC services for effective hypertension and diabetes management. The project has implemented several critical interventions to enhance hypertension and diabetes management across three states. The project focuses on improving supply chain management through advocacy for ready reckoners, maintaining drug registers, eAushadhi-based stock analysis, and estimating drug requirements. NCD programme reviews are conducted regularly, integrating key metric indicators and developing factsheets with district rankings. Strengthening NCD portal use involves advocacy and handholding for real-time data entries, population and ABHA seeding, routine data analysis, and addressing zero-data entries.
Impact:
- A detailed Baseline Assessment Report was developed to evaluate the existing landscape.
- As part of the capacity-building efforts, 793 Medical Officers and 2,964 Community Health Officers have been trained.
- Cascade training sessions and on-the-job support provided focusing on blood pressure measurement protocols
- 200 supportive supervision visits conducted during October 2024 to April 2025
